Refund policy

This product contains a 30 day cancellation period. Members receive a full refund of membership fees, excluding registration fee, if membership is canceled within the first 30 days after the membership effective date.

CANCELLATION POLICY: Members identified as the primary account holder may cancel their membership any time by calling at (954-874-9602) or by providing a signed letter of cancellation notice including membership number to SIMPLE HEALTH at the following address: FORT LAUDERDALE, FL 33306 or via email at: cs-simplehealth@simplehealthplans.com.

If member cancels within THIRTY (30) days of signing up for an initial membership we will refund the first month's paid membership fee. UNLESS MEMBERSHIP CANCELLATION NOTIFICATION IS RECEIVED BY SIMPLE HEALTH AT LEAST ONE BUSINESS DAY PRIOR TO ANY MONTHLY RECURRING CHARGE DATE MEMBERS UNDERSTAND AND AGREE THAT MEMBERSHIP WILL AUTOMATICALLY CONTINUE AND MEMBER AUTHORIZES US TO COLLECT THE THEN APPLICABLE MEMBERSHIP FEE (WITHOUT NOTICE TO MEMBER, UNLESS REQUIRED BY APPLICABLE LAW), USING THE PAYMENT MECHANISM PROVIDED WHICH WE HAVE ON RECORD FOR THE MEMBER.
